Patient Portal - Terms & Conditions
All registered users of the Patient Portal are subject to the terms and conditions of use below.  Please read the terms of use carefully before accessing or using any part of the Patient Portal.  By accessing or using the Patient Portal, you agree that you have read, understand and agree to be bound by these terms of use.  It is important to review the terms of use periodically as we may modify them at any time, and you agree that such modifications are effective and binding upon you immediately upon posting of the modified version.  If you do not wish to agree to the terms of use, do not access or use any part of the Patient Portal.
Specifically you acknowledge and agree to the following:
  1. You agree that you will not share your User ID and Password with anyone unless specifically authorized in writing by Garrett Medical Group, PA (GMG) to allow a family member or legal representative to have access to your Patient Portal account.
  2. You understand that the Patient Portal cannot be used for urgent or emergency matters.  You should contact 911 for emergencies.
  3. Your messages may become part of your medical record and therefore such messages will be secured, accessible and disclosed in the same manner as the rest of your medical record (e.g. such information may be disclosed to your insurance company if they are conducting an audit or validating a claim).
  4. You will inform GMG if you change your email address.
  5. You will inform GMG if you lose your password or if you believe that there has been unauthorized access to your account on the Patient Portal.
  6. If GMG determines that you have violated this agreement and/or abused the use of this service, we reserve the right, at our sole discretion, to discontinue your use of the Patient Portal.  You will be notified if your Patient Portal service is discontinued.
  7. GMG providers will only email you at the email address you have provided.  Please note that the Patient Portal messages may be viewed by other health care providers, administrator and others who work with your provider and his/her practice.  You should not use the Patient Portal for highly sensitive health information such as AIDS/HIV, mental health or substance abuse treatment information.
  8. The Patient Portal is not a substitute for timely contact and consultation with your provider.  You should never change or stop any course of treatment ordered by your provider before asking him or her.  Also, secure messages sent by you must have specific and detailed requests/questions.  If not enough information is given in your message, we may not be able to help you.
GMG and its affiliates, suppliers, and other third parties mentioned on this site are neither responsible nor liable for any direct, indirect, incidental, consequential, special, exemplary, punitive, or other damages (including, without limitation, those resulting from lost profits, lost data, or business interruption) arising out of or relating in any way to the site, site-related services and products, content or information contained with the "site," and /or any hyperlinked web site, whether based on warranty, contract, tort, or any other legal theory and whether or not advised of the possibility of such damages.  Your sole remedy for dissatisfaction with the site, site-related services, and/or hyperlinked web sites is to stop using the site and/or those services.  Applicable law may not allow the exclusion or limitation of incidental or consequential damages, so the above limitation or exclusion may not apply to you.
